"Select Items Only" - coll mentioned the same thing last year - that's not the same thing as the insert postcard.
I remember two times where there was no monthly coupon card, with the most recent being the one where there was an issue with printing this past summer. Pretty sure the other vacant month was last November.

I used last year's November code on Chewie, it was a code like any other $20 code.

I definitely remember the "error at the printer one" but I haven't been around long enough to remember any others. I think since July 2014 I managed to use every single code except HEAT14 as I didn't know about this thread and the cancel reorder method.
